How do I submit dental premiums to my MN support case account on a recurring basis? I can’t find any information on how to handle the “premiums” portion and how to calculate the actual amount eligible for reimbursement.

You really need to contact the case worker. Make sure you get her name and direct phone number. If she does not help you, I’d advise speaking with the head of the Department. ALWAYs make them tell you their names and keep that for your records.
